A free workshop for health and community service providers, primary health services and acute care staff. 

10am - 3pm
Morning tea provided
BYO Lunch

Workshop Outcomes:

· Have a better understanding of the principles of advance care planning
· Be familiar with the Tasmanian Advance Care Directive
· Be familiar with other resources available to support your practice
· Be able to explain advance care planning to others
· Understand substituted decision making options in Tasmania
· Be able to discuss end of life wishes and advance care planning with  clients and consumers
  Feel more confident responding in difficult conversations
· Have a better understanding of the needs of specific communities in Tasmania  (including CALD, Aboriginal, LGBTIQ  and homeless people)  when considering end of life preferences.

These workshops will include interactive group discussions.  There will be practical learning based on case
scenarios in having discussions of concerns and wishes for end of life care.
